What is Root Cause Analysis?
Root Cause Analysis (RCA) is a structured, investigative approach used by individuals or teams to identify the underlying reasons or origins of a problem.
By digging deep into the root causes of a problem rather than just addressing its symptoms, RCA aims to prevent the problem from returning in the future.
This systematic approach involves following scientific thinking--gathering facts and data through direct observation, using quality tools to establish cause-and-effect relationships, and determining the the dominant causes of the problem through in-depth analysis.
